WHAT YOU WILL DO
As a Staff Nurse, you will deliver safe and therapeutic nursing care using the Nursing Process and in accordance with the Nurse Practice Act, DOH Regulations, State Regulations and the policies and procedures of Fair Acres.
Responsibilities:
- Is confident and flexible in ability to practice nursing on any unit throughout the facility, and maintains a clean, safe, and orderly environment.
- Maintains open, professional communication with the Unit Manager, Nursing Supervisor, residents, visitors and employees.
- Supports the goals and objectives of the Nursing Department and Administration of Fair Acres and participates on committees as requested.
- Ensures effective organization of assignments by establishing priorities and setting goals.
- Evaluates and reports all significant symptoms, reactions, and changes in a resident’s condition.
- Records pertinent information and assures the continuity of accurate and timely documentation to facilitate communication among services.
- Performs nursing functions, such as:
- Administration of medications and treatments
- Activities of daily living
- Rehabilitative techniques to restore and/or maintain resident independence
- Maintains standard precautions.
- Provides end of life care.
- Participates and monitors the TAPS program.
- Collaborates with members of the interdisciplinary health care team in providing quality care, and documentation of resident goals.
- Promotes and models a cooperative team effort among staff and peers.
- Maintains and ensures resident and staff confidentiality to be compliant HIPAA.
- Participates in Staff Development programs as mandated by Fair Acres and seeks additional self-enrichment and professional growth through continuing education.
- Ensures correct use of equipment and supplies, and notifies appropriate departments when maintenance is required.
- Assumes other duties as deemed necessary by the Unit Manager and/or Nursing Supervisor.
WHAT YOU NEED TO BE SUCCESSFUL
- Graduate from an accredited nursing program
- Current Pennsylvania RN license
- New grads encouraged to apply for full-time positions.
- Per Diem nurse role requires minimum 1 year acute or long-term care experience

Position Summary
The Resident Nurse Assessment Coordinator objective is the achievement of the highest quality resident care thru coordination and input according to the RAI Manual and CMS and State guidance along with adherence to the established procedures of the assessment process.
Responsibilities
- Accurate and timely completion of RNAC designated sections of the MDS on assigned units
- Assure completion of the MDS by other disciplines and report non-compliance to Director
- Timely signing of all necessary paperwork and coordinator of IDC meetings
- Attend scheduled MDS/IDC meetings on assigned units or where needed
- Participate in the evaluation process to determine the need for a Significant Change and implement Significant Change MDS and IDC meeting
- Accurate and timely scheduling and completion of discharge assessments and Death in Facility and entry trackers
- Assist in determining Medicare A Nursing skill upon admission and readmission; notify IDC team and RAD Department when skill ends
- Assist in completion of required documentation
- Oversight of Care Plan completion and timely updates
- Notify IDC team of changes to ARD or IDC date
- Provide information for Managed Care updates and audits as requested
Qualifications
- Graduate of an accredited school of nursing
- Current PA Registered Nurse License
- 1–2-year experience in Long Term Care
- Previous Registered Nurse Assessment Coordinator experience preferred

As Nursing Supervisor, you will oversee staff to ensure the delivery of safe and therapeutic nursing care using the Nursing Process and in accordance with the Nurse Practice Act, DOH Regulations, State Regulations and the policies and procedures of Fair Acres.
Responsibilities:
- Supports the facility mission and culture change.
- Customer Service focused. Meets, talks to and builds relationships with residents, families, and employees on routine basis to identify areas of concern, implements measures to meet needs, and evaluates ongoing satisfaction.
- Performs routine rounds with employees to assure accountability, provide education where needed and offer positive reinforcement when warranted.
- Promotes a team environment.
- Meets routinely with new employees during probationary period to evaluate performance and to help in areas identified.
- Routinely evaluates the work site to assure that employees have the equipment they need to perform their job in a safe environment.
- Evaluates that employee are trained and competent in the operation of equipment required for their job responsibilities i.e. Factcheck procedures, lifts, etc.
- Reviews various reports and utilizes the information to evaluate appropriate care and services.
- Participates in monthly meetings held by Unit Mangers with all employees, pertinent information related to the facility, department, job function, status of unit etc. and solicits input to improve the outcomes for the residents and employees.
- Participates in committees.
- Communicate critical issues with Nursing Administration on routine basis regarding issues, concerns, or challenges and intended plan of action to correct outstanding issues.
- Routinely audits and evaluates outcomes of high risk, high volume and problem prone procedures, ie. Medication passes preventative skin care, fall, etc.
- Coordinates with the interdisciplinary team
- Duties as assigned.
WHAT YOU NEED TO BE SUCCESSFUL
- RN from an accredited School of Nursing, BSN preferred.
- PA RN License
- 3 Years in Long Term Care; supervisory experience preferred.

WHAT YOU WILL DO
Under the direction of the Food & Nutrition Supervisor or Cook Supervisors. Responsible for all food preparations and service, daily sanitation duties and assignments.
- Assists in preparing food and serving of same.
- Assists in pre-setting, breakdown and cleaning all dining room tables, food carts and steam tables.
- Preparation of food items for next day’s meal as assigned, signature required upon completion. Responsible for accuracy of all items that are prepared.
- Responsible for cleaning and sanitizing as you go; all worktables, tray line stations, equipment, carts, floors, pot, pans, etc.…and all assigned special cleaning duties.
- Maintain all storerooms in neat, orderly and clean fashion. Responsible for checking in all deliveries to kitchens off order/delivery sheets. Order/ delivery sheets will then be initialed after items are checked indicating accuracy of deliveries. Deliveries will be stocked in their respective areas.
- Responsible for sanitation of all floor areas: sweeping, mopping, and buffing of same.
- Operation of dish machine, washing/rinsing/sanitizing procedures for all kitchen utensils.
- Responsible for maintaining all dock area in a neat, orderly and clean fashion.
- Responsible for disposing of all trash items, breaking down boxes and depositing these items into designated trash dumpsters.
- Responsible for properly soaking, washing, sanitizing, and wiping all silverware and china items
- Delivery of all Food & Nutrition Food carts individual and bulk nourishments to each resident unit and retrieving the same.
- Responsible for preparing individual resident nourishments according to the nourishment logbook
- Responsible for Fair Acres and/or outside special function preparation, set-up, serving and clean up as assigned.
- Responsible for maintaining all Food Service areas according to policies, procedures and standards set by the Food and Nutrition Department.
- Serve residents meals in an efficient and courteous manor
- Physical Demands
- Ability to lift to 30 pounds; stand for up to 4 hours at a time.
Requirements
- Must be Delaware County Resident
- High School Diploma or equivalent required
- Ability to follow oral and written instructions.
- Ability to know the operation of convection oven, steam tables, refrigerator, trash compactors and freezer units.
- Ability to communicate effectively with personnel from various departments.
