1. For Caregivers

As we age, our bodies and minds evolve, bringing new challenges and opportunities. At COSA we are here to support you on this journey with resources, advice, and community. Whether you are embracing new milestones or navigating the complexities of growing older, we understand the importance of compassion, dignity, and joy at every stage of life.  We have a variety of programs and services meeting the needs of the very healthy to those needing in home care and we are here to help connect to whatever you are looking for yourself or for your loved one.

Assessment is the first step in the application process for programs and services. A certified assessor will complete an assessment and determine a level of care. For an In-Home Services Assessment or Nursing Home Assessment application packet, call the Intake Specialist at 610-490-1300.
